Same-Day Crowns: How CEREC Works, and When a One-Visit Crown Makes Sense

By Dr. Hamid Nasr, D.D.S. 4 min read

For most of dentistry's history, getting a crown meant two appointments a couple of weeks apart: one to prepare the tooth and take a putty impression, then a stretch with a plastic temporary while a lab made the crown, then a second visit — and a second round of numbing — to cement it.

CEREC changes that. At Creekside Dentistry in Folsom, most crowns are designed, milled, and placed in a single appointment. Here's what actually happens, and when a same-day crown is (and isn't) the right call.

Why a tooth needs a crown

A crown covers a tooth that's too damaged for a filling to hold up on its own. The most common reasons:

  • A large old filling that's cracking or leaking
  • A cracked or broken tooth
  • A back tooth after a root canal, which has lost structure and is more prone to fracture
  • Heavy wear from grinding
  • Replacing an old crown that has failed

What CEREC is

CEREC stands for Chairside Economical Restoration of Esthetic Ceramics. In plain English, it's an in-office digital system with three parts: a scanner that captures your tooth in 3D, design software, and a milling unit that carves the crown from a solid block of ceramic. At Creekside, crowns are milled on a CEREC Primemill right in the office.

Your same-day crown visit, step by step

  1. 1Numbing and preparation. Dr. Nasr removes decay and old filling material and shapes the tooth, just as for any crown.
  2. 2A digital scan instead of putty. A small camera wand captures the prepared tooth, its neighbors, and your bite. No gagging on impression trays.
  3. 3Design. The crown is designed on screen to match your bite and the shape of the surrounding teeth. Many patients like watching this part.
  4. 4Milling. The design is sent to the mill, which carves the crown from a ceramic block, usually in under half an hour.
  5. 5Finishing. The crown is checked, adjusted, and polished or glazed and fired so it blends in with your other teeth.
  6. 6Bonding. The finished crown is bonded to your tooth. You leave with your permanent crown.

Plan on roughly two hours in the chair for a single crown, depending on the tooth.

The advantages

  • One visit, one numbing. No second appointment, no second injection.
  • No temporary crown. Temporaries can come loose, feel bulky, and let sensitivity creep in. With CEREC you skip that stage entirely.
  • Precise digital fit. The scan and design are highly accurate, and the margin can be checked on screen before milling.
  • Strong, natural-looking ceramic. Modern milled ceramics are tough enough for back teeth and can be shaded to match.
  • Less time off work. One appointment instead of two.

When a lab crown may still be the better choice

Same-day technology covers the majority of single crowns, but not every case. A traditional lab-made crown may make more sense when:

  • A front tooth needs complex, hand-layered porcelain to match a very distinctive neighboring tooth
  • Many teeth are being restored at once as part of a full-mouth or bite reconstruction
  • A gold crown is the best material for a particular situation
  • Gum tissue needs time to heal before a final impression

Dr. Nasr will tell you which approach fits your tooth and why.

Do same-day crowns last?

Yes. Longevity depends far more on the health of the tooth underneath, your bite, and home care than on whether the crown was milled in the office or made in a lab. Brushing twice a day, flossing, regular cleanings, and a night guard if you grind give any crown its best chance.

Does insurance cover a same-day crown?

A same-day crown is billed like any other crown, so your plan's crown benefit generally applies. That's subject to your plan's limits, waiting periods, and any "alternate benefit" clause that pays a lower rate for ceramic crowns on back teeth. Our front desk can check your benefits before your visit.

Same-day crowns in emergencies

Same-day technology matters most when something breaks. A cracked or broken tooth can often be scanned, milled, and crowned in one sitting, so you don't spend weeks protecting a fragile tooth with a temporary. If you've broken a tooth, call the office and we'll tell you how soon we can see you.

If a dentist has told you that you need a crown, or you have an old filling that's cracking, we can usually evaluate it and often complete the crown in the same visit.
