What this website collects
The appointment-request form collects the information you type into it: your name, phone number, email address, preferred days and times, preferred language, and any notes you choose to add. Nothing is collected silently, and the form does not ask for — and should not be used to send — detailed medical information. Clinical details are discussed privately, at the office.
How it’s used
Your request is delivered to the Creekside Dentistry front desk and stored securely so the office can call you back to schedule. It is not sold, shared with advertisers, or used for marketing lists. A confirmation email is sent to the address you provide.
Cookies & analytics
This site does not use advertising cookies or cross-site tracking. Anonymous, privacy-respecting page-view counts may be collected to understand which pages are useful. The embedded Google Map on the contact page is served by Google and is subject to Google’s own privacy policy.
Your records as a patient
Clinical and billing records are governed by HIPAA and handled within the practice’s own systems — this website never touches them. To request, correct, or ask questions about your records, call (916) 983-8777 or ask at the front desk.
