Dr. Nasr has practiced dentistry for 30 years — 23 of them at the same Folsom address. That kind of tenure changes how a dentist works: he answers to neighbors, not strangers, and the next checkup matters more than the next procedure.
For the past 13+ years he has specialized in implant dentistry, planning every case on 3D I-CAT imaging and carrying it from placement to final crown himself, as a member of the American Academy of Implant Dentistry.

Dr. Sholeh Rahimi, M.D., Ph.D. is a board-certified internal medicine physician. She earned her M.D. (1994) and Ph.D. (1997) at the University of Freiburg in Germany, completed her internal-medicine residency at the University of Nevada School of Medicine, and is certified by the American Board of Internal Medicine. She practices at this same address, and is fluent in English, German, and Persian.
Together, she and Dr. Nasr are Creekside Medical Dental — a rare model where dental and medical care share one roof.
